Position Summary
Under the supervision of the Social Work Supervisor, the Social Worker Assistant provides social-service and care-management support to participants of Hong Fook Adult Day Health Care Center. The position assists with prospective-participant intake, home visits, enrollment, language assistance, community referrals, interdisciplinary coordination, outreach, participant adjustment, and psychosocial assessment and documentation under appropriate direction.

Essential Duties & Responsibilities

• Introduce Hong Fook Adult Day Health Care programs to prospective participants, families, caregivers, referral sources, and community partners.
• Support intake activities, including home visits, initial information gathering, participant screening, and completion of assigned assessment documentation.
• Coordinate follow-up needed to bring prospective participants in for program assessment, including obtaining medical reports, scheduling transportation, tracking documentation, and notifying appropriate team members.
• Support participant enrollment by explaining program rules, services, schedules, and participant rights and helping new participants adjust to the program.
• Provide assigned care-management support, monitor service needs, follow up on referrals, and promptly report participant concerns or significant changes to the Social Work Supervisor and appropriate team members.
• Help participants and families obtain needed community services, public benefits, health coverage, and other resources and serve as a liaison with authorized outside agencies.
• Explain and interpret service, benefit, and health-insurance notices for participants and families within the employee’s knowledge, language proficiency, and assigned scope; refer eligibility or legal determinations to appropriate resources.
• Assist the Social Work Supervisor with initial and periodic psychosocial assessments, care planning, reassessments, and maintenance of complete, accurate, timely, and confidential records required by the program.
• Assist with or co-facilitate participant support groups, educational activities, and other social-service programming under appropriate direction.
• Participate in and help coordinate outreach activities for frail older adults, adults with disabilities, families, caregivers, and community partners.
• Participate as a member of the interdisciplinary team and communicate relevant participant information, needs, barriers, and follow-up status.
• Conduct assigned local travel and home or community visits and follow applicable safety, mileage, and confidentiality procedures.
• Perform other related duties as assigned by the Social Work Supervisor.

Minimum Qualifications

• Bachelor’s degree in social work or a related human-services field required.
• Experience or demonstrated interest in working with frail older adults, adults with disabilities, families, or community-based health and social-service programs.
• Ability to work independently and collaboratively as a member of a diverse interdisciplinary team.
• Strong oral and written communication, interpersonal, organization, follow-up, customer-service, and time-management skills.
• Ability to manage multiple assignments, maintain accurate records, recognize matters requiring escalation, and protect participant confidentiality.
• Ability to provide community presentations and assist with outreach activities is preferred.
• Bilingual proficiency in English and Cantonese and/or Mandarin is preferred. Proficiency in Vietnamese, Filipino/Tagalog, Korean, or another Southeast Asian language is also desirable.
• Ability to work a flexible schedule when necessary to meet participant and program needs.
• Valid California Driver’s License, a driving record acceptable to the agency’s insurer, and reliable access to an automobile for assigned travel are preferred.

Working Conditions
Adult day health care, office, participant-home, outreach, and community environments with frequent participant and family interaction, interdisciplinary collaboration, documentation, computer work, and possible local driving. Work schedules may occasionally vary based on participant and program needs. Reasonable accommodation may be made for qualified individuals with disabilities.

Employment Conditions

• Orientation Period: This non-exempt position includes a three-month orientation period beginning on the employee’s first day of employment.
• Health and Compliance Requirements: Employment is subject to completion and maintenance of health screenings, background clearances, training, driving requirements, and other qualifications required by law, regulation, funding source, insurer, or Family Bridges policy.

• Ability to sit, stand, walk, bend, reach, and perform computer, documentation, meeting, outreach, and participant-service work.
• Ability to travel to participant homes, community locations, and Family Bridges sites as assigned.
• Ability to lift and carry items weighing up to 20 lbs., with or without reasonable accommodation.
